The application of sildenafil in preventing pregnancy

Sildenafil, also known as sildenafil (commonly known as Viagra, trade name Viagra), is a drug accidentally invented during the development of cardiovascular disease drugs to treat male erectile dysfunction. It is generally known under its commercial name Viagra.
The role in reproduction
Due to the distribution of phosphodiesterases (PDEs) in various tissues such as corpus cavernosum smooth muscle and uterine artery smooth muscle, and the presence of NO synthase in the uterus, sildenafil not only acts on corpus cavernosum smooth muscle and blood vessels, but also on uterine artery smooth muscle, increasing blood flow in the uterine artery. At the same time, by expanding the spiral artery of the endometrium, the spiral artery of the endometrium becomes thicker, leading to increased blood flow, capillary dilation, increased blood flow, and thickening of the endometrium. This significantly reduces the Pl and Rl values of the uterine and ovarian arteries in patients, increases blood supply to the uterus and ovaries, and is beneficial for pregnancy. Jerzak et al. used sildenafil to treat patients with recurrent miscarriage and found that sildenafil can reduce the activity of NK cells and improve the immune environment of the endometrium. 
Therefore, sildenafil can effectively reduce uterine artery blood flow, promote endometrial growth, and improve the immune environment in the uterus.
Indications
1. Increased uterine artery resistance during early pregnancy:
Vaginal administration, sildenafil is generally 50-100mg daily, once a day, for 7 consecutive days;
Follow up with vaginal ultrasound after treatment to monitor changes in uterine artery blood flow pulsatility index and blood flow resistance index.
If there is a significant decrease in the resistance index of uterine artery blood flow, it can be reduced, and it can be discontinued after a week of follow-up if it is still normal.
If sildenafil is ineffective, it can be changed to nifedipine controlled-release tablets, which are effective in reducing uterine artery blood flow resistance but do not promote endometrial growth.
It can be combined with mild compound ass hide glue paste or compound salvia miltiorrhiza Chuanqiu granules, etc. If necessary, it can be combined with nifedipine controlled release tablets, etc., but it is not recommended to combine with nitrates.
2. Infertility caused by thin endometrium:
Vaginal administration of sildenafil 100mg/d, or oral administration of Bujiale combined with vaginal administration of sildenafil. The combination therapy has a significant effect on infertility caused by thin endometrium, which can increase local blood circulation, increase pregnancy rate, and increase patient satisfaction.
Improving the endometrial receptivity of patients after intrauterine adhesions surgery, especially for patients with moderate to severe intrauterine adhesions, can significantly increase the pregnancy rate of frozen thawed embryo transfer;
Improve endometrial blood flow in patients with repeated transplant failures and increase clinical pregnancy rates.
